Step 1: The document checklist

• Your coloured Passport-sized Photograph that has been self-attested
• Please carry a copy of your Proof of Identification (POI):
  PAN Card or
  Driver’s License or
  Voter’s ID Card or
  Passport
• And also a copy of your Proof of Address (POA):
  Ration Card or
  Driver’s License or
  Voter’s ID Card or
  Passport

*Please carry the original document(s) for verification

Step 2: Decide what you want

• MTS Connection with Handset        
  Available Handsets with MTS
• MTS Connection without Handset  
  MTalk

Step 3: Find an MTS Retailer

• To locate an MTS Retailer call us at 9151-155-155 for Uttar Pradesh East and 9152-155-155 for Uttar Pradesh West
  (Don’t forget to bring all the documents mentioned above)

Step 4: Fill in the Consumer Application Form (CAF)

Step 5: Consider Recharge Options

Your SIM will contain a certain minimum talktime, so you may wish to purchase a Recharge Voucher to top-up your balance:
